Problem Statement

Write a 1500-word essay addressing each of the following points. Be sure to completely address each bullet point. Separate each section in your paper with a clear heading that allows your professor to know which bullet you are addressing in that section of your paper. This week you will develop a problem statement about the Healthcare issue identified at your practicum site. A problem statement is a concise description of an issue to be addressed or a condition to be improved upon.

· Your problem statement will identify the gap between the current state and desired state after the implementation of the evidence into practice.

· Provide any reports or currently available data to document the need identified by your practicum site.

· You will peruse the literature to examine how a similar problem has been addressed.

· Provide at least four (4) references ( within the past five years, 2020-2025) to support a proposed evidence-based intervention.
